Got my OLED. I’m a little confused on the 90hz refresh rate. Is it best to just leave the slider at 90hz/fps? What happens when a game isn’t hitting 90 FPS. What’s best for the smoothest experience?

You don’t want to keep it at 90 unless the game is hitting 90 fps in your counter. Typically for indie style games I put it at 45 and the SD doubles up frames to 90 and it looks smooth. For AAA games I put it to 30 and the SD triples up frames.
