Unfortunately my apple watch S9 only turned on the hand washing timer once despite all of the options being activated and never after that. What is the problem? Why is this feature so inconsistent?

Because it sucks, nobody uses it, and Apple knows this and doesn’t want to waste time and resources working on it.
Uh oh, guess we’ll have to go back to singing “happy birthday to you”.
(Seriously though, who even uses this feature?)
